Dr. Eleazar Tayag is a recognized leader in the home health and hospice space, serving as the CEO of Imperial Home Health and COO of Imperial Hospice and Palliative Care. With a focus on delivering exceptional patient care, he has championed innovative programs like rehospitalization prevention and fall prevention, redefining care standards in health care.As the founder of Imperial Physical Therapy P.C., Dr. Tayag continues his mission to provide comprehensive healthcare at every stage of life. Beyond his organizational leadership, he is deeply committed to advancing the profession of physical therapy on a global scale. Dr. Tayag serves as the President of FUTURE Foundation Global, a leadership incubator for Filipino physical therapists worldwide. He also actively contributes to the American Physical Therapy Association (APTA), championing initiatives that improve access to care and enhance patient outcomes.
Dr. Tayag holds advanced certifications, including being a Board-Certified Clinical Specialist in Geriatric and Orthopaedic Physical Therapy, a Certified OASIS Specialist – Clinical (COS-C), and a Board Certified Healthcare Executive (BCHE). His research and advocacy focus on geriatric care, home modifications, and sustainable healthcare solutions.

Moving from Seattle, WA, to join Imperial Physical Therapy P.C. in Victorville, CA, Dr. Tayag brings a wealth of knowledge in outpatient rehabilitation.
